I will show how to create simple employee survey using Microsoft forms and how easily you can embed it inside Microsoft teams. so let's not waste any time and jump right into it. I am inside office 365 environment so this is Office 365 application page from here I will click on the forms application it will take me to the forms.office.com just a welcome message here you can see there are there is an employee survey already created so this is created by default but Microsoft when you launch this application you can enhance that you can use it I'm gonna do I'm gonna click on a new form to create a new survey

I will give this form a title I will just call it the Annual survey. now I will add field so I will click on add new and select the chosen field and I'm asking a question to my employee are you happy with your job? for example, so suggested options could be yes, no I can add more options, not sure. I will add a new column this time a choice again and I will ask the second question to employees. Do you have work/life balance?
now the options could be yes/no or other options could be not sure. so you can add as many questions you want this is just an idea to give you how you can create Microsoft Forms. so once you've done it you can click on the preview on the top and see the preview of this form, now if Igo back I can see the response is at the moment that no response is submitted I can click on the share and share this Form in a many different ways so I can share it as a link so I can share as a code so which people can scan on their mobile device it on this phone or I can embed it in a web site or I can click on the email and email this link straight to someone else. I want to embed this inside Microsoft teams,
so I will choose none of these option I want my employees to reply inside Microsoft teams to this survey and also to see the results inside microsoft teams so now if I go back to the forms I can see the annual survey form is created there if I never get back tothe Microsoft Teams now I have a team called everyone so all my employees out there in that team so if I click on the plus link from this team's tab menu andI can search for it or I can click on the forms option and I will click on add an existing form because this form is already there no it is giving me all the forms available in my Tenant so I will click on an annual survey and there are different type you can click responses or show results but this time I will collect responses and then I will just hit save and post this to a channel about this tab so everyone knows that the form is there to respond.
I can see the form tab and in the post section I can see a post about this form as well I can go back to the form and now maybe I fill this form say yeah I'm happy with my job and yes I've worked my balance so you can see Thanks message once you submit it I will do another submission so this time not sure and no two responses are submitted now. I can go to a plus tab again and now this time I can see the results so I will again select the form and also sure it is time show result instead of collect responses and I will save it.
so this time it will show me all the results of the survey all the responses rather than the form itself I will just go back in once in the form and submit once more time and submit the third response now if I go to result in tab I can see the summary of the form is there I have three responses from three different employees now if I navigate back to the Microsoft forms and I click on the entry form and click on the responses I have to refresh the page to see the latest responsive. now you can see that from within the Microsoft forms
